Progression of symptoms and clinical measures in brain-first and body-first Parkinson’s disease subtypes

Project description

Parkinson’s disease (PD) is a clinically and biologically heterogeneous neurodegenerative disorder. To explain this variablity, we have proposed two PD subtypes: brain-first, where pathology initiates in the central nervous system, likely the olfactory bulb, and body-first, where α-synuclein pathology begins in the peripheral autonomic nervous system and spreads to the brain. This distinction may reveal differences in underlying pathobiology, disease progression, and treatment response.

This PhD project aims to longitudinally characterize motor and non-motor symptom progression across these subtypes. Using data from nearly 200 newly diagnosed PD patients in out patient cohorts at Aarhus University Hospital, we will identify clinical patterns that differentiate brain-first and body-first trajectories. Subtyping is based on diagnostic markers such as polysomnography-confirmed REM sleep behavior disorder and cardiac [123I]MIBG scintigraphy.

A particular focus will be placed on autonomic dysfunction, cognitive decline, mood disturbances, and postural instability. We hypothesize that body-first patients will present with more severe non-motor symptoms at diagnosis and worse cognitive performance, while brain-first patients may show more rapid decline in these domains over time. Additionally, the project will investigate pupillometry as a potential bedside tool for assessing autonomic function and supporting subtype classification.

By integrating multimodal data including PET, MRI, autonomic testing, validated clinical scales, and digital biomarkers, this project aims to refine subtype-specific disease modeling. The outcomes are expected to enhance clinical prognostication, support individualized treatment approaches, and contribute to the development of more targeted disease-modifying therapies in PD.

Qualifications

Professional experience with Parkinson's disease. Must be an M.D.
